The info you have is 100% correct and should be easily accomplished. The "swift" ID makes it easy as pie for any bank to wire direct to Jack's Bank. $45 is outrageous and should be credited back to you since the wire was not completed. I'm sure you already did this but, don't deal with ateller, ask to speak with the operations officer. He or she should likely have experience at a larger bank and have a contact to call that can tell them how to get this simple transaction done.
